I am in the hospital after having my thyroid gland removed due to a malignant tumor. What documents and who should issue them to me in order to appear before the OLKK commission for extending sick leave?
Can I receive more than one referral from the general practitioner within one calendar month for different specialists? Is it necessary to go for examinations that will issue me additional documents for the committee to extend sick leave?

The procedure by which providers of primary outpatient care (general practitioners) and specialized outpatient care (specialists), who have signed contracts with the National He alth Insurance Fund, carry out a medical examination of temporary incapacity;
referral for medical examination to the LCC; preparation of documents for presentation to TELK is bound to the activity necessary to carry out examinations and consultations with a relevant specialist or to the LCC (when the regulations provide for the medical expertise to be carried out by the LCC), as for this purpose the general practitioner or outpatient care specialist issue:
“Medical examination voucher” (Bldg. MOH-NHOC No. 6); directions for medical-diagnostic activities (Bldg. MOH-NHOC No. 4); and in certain cases - a referral (Bldg. MOH-NHOC No. 3A) if highly specialized medical activities are necessary.
The general practitioner issues a "Medical Examination Voucher" (Block MH-NHOC No. 6) to the LCC, when an extension of the temporary incapacity for more than 14 days is required, according to the provisions of the Ordinance on the Medical Examination and the Regulations for the Device and the organization of work of the bodies of medical expertise and of the regional files of medical expertise.
During the preparation for LCC, the patient is referred for the necessary examinations to specialists from medical institutions for outpatient care, who have signed a contract with the NHSOC, with a "Medical Examination Voucher" (Bldg. MH-NHSOC No. 6).
If the LCC considers that additional tests or consultations are necessary, it appoints them with a referral for medical-diagnostic activities (Block MOH-NHOK 4) and/or with a "Medical Examination Voucher" (Block MOH- NHSOC 6).
When the regulations provide for the medical examination to be carried out by TELK, the LKK prepares the patient's necessary documentation and directs him to the TELK authorities.
In the case of permanently reduced working capacity, which is not preceded by temporary incapacity, the patient's personal physician prepares the documents for presentation to TELK and directs him with a medical referral for TELK, and this referral is not the above-mentioned "Receipt for medical expertise" (Bldg. MH-NHOC No. 6), and direction according to the Regulations for the organization and organization of work of the bodies of medical expertise and of the regional files of medical expertise.
