I have an appearance before the Commission for certification of work capacity. Please clarify, after receiving a letter from TELK, with instructions on what examinations and tests I should submit to them, what documents the personal physician should issue to me?

In the event that for the needs of the medical examination of working capacity, in the part of preparation for re-certification by TELK (NELK), examinations and consultations with a relevant specialist are necessary - the personal physician or specialist, or the LKK issues to the he alth insurance company the "Coupon for medical examination" (Bldg. MOH-NHOC No. 6).
If specialized, highly specialized medical-diagnostic tests or highly specialized medical activities are required, respectively, the personal physician or specialist, or LKK issues the following documents to the he alth insurance:
• "Department for medical-diagnostic activity" (Bldg. MOH-NHOC No. 4);
• "Department for medical-diagnostic research" (Bldg. MOH-NHOC No. 4);
• "Medical direction for highly specialized activities" (Bldg. MOH-NHOC 3А);
• "Medical department for highly specialized activities" (Bldg. MH-NHOC No. 3A) only for HCVs included in the dispensary monitoring of the patient when he is dispensated by the personal physician, and HCVs from the "Anesthesiology and intensive care" package”
When preparing for TELK (NELK) (for certification or re-certification), medical facilities carry out activities (medical consultations, examinations and issue documents) according to the specific diseases and within the scope regulated in Ordinance No. 9 of 2019
When preparing for TELK (NELK) for certification or re-certification, doctors in medical facilities may apply medical consultations, epicrisis and research carried out on another occasion in the last 12 months and regulated in Ordinance No. 9 of 2019. After submitting the documents to TELK (NELK), at the request of the latter, only VSMDI with "Direction for medical-diagnostic research" (Bldg. MH-NHOC 4) are appointed.
