Now it is most likely a somatic, so-called masked depression. I.e. when a mental problem is masked behind the facade of various physical problems.

Masked depressions are depressive states, regardless of their origin, in which the somatic symptoms are so pronounced that they completely hide the depressed. Patients turn to a doctor because they have cardiovascular complaints, headaches, altered sleep, a feeling of internal tension, a feeling of suffocation, stabbing in the chest.

In all cases, however, with a longer conversation and directing the questions to the sphere of how the patient evaluates himself, a decrease in the level of interests, a loss of the previous vigor, a tendency to withdraw into himself, a decline in the ability to enjoy things that previously gave him joy. Since the diagnosis of masked depression is not nosologically specific, it is very important, after it is made, to make the nosological diagnosis, which will also determine the way of further treatment.

With masked depression, there is a real danger of suicide, and therefore the clinical and social significance of its unmasking is particularly great. That is why, when masked psychopathology is suspected, the best treatment is the complex one, which combines psychotherapy with psychotropic pharmacotherapy and medications aimed at influencing the symptoms themselves. It can also be started only with psychotherapy or the appointment of antidepressants and anxiolytics.
